2. I'd drop the UVB light down below 18" and when was the last time it was changed? They only last 6 months then they need to be changed.

4. Find a smaller room and keep the door shut, if you have to, purchase a electric floor heater for that room and get the temps up to mid 80's during the day. Put a towel down on the floor in front of the door, this should help keep heat in the room. IMPORTANT note: install a smoke detector in the room too, as a safe guard.
