Toes falling off

I have a friend with a baby iguana who keeps losing its toes. He has a full spectrum bulb with a reflector, heat rock and plywood for the floor in a 10 gallon aquarium. She is feeding him greens and supplementing with calcium and a vitamin spray. Any ideas? Thanks for your help. traci
