Sulcata and Cactus? To clean or not to clean that is the???

Ok, everyone tells me to clean my cactus pads of the spines (large or small) before I feed them to my sulcatas. I wind up with 25% of the pad left and more prickers in my fingers than I ever saw on the cactus pad. I am curious who does this for the tortoise out in the wild? My question is: Is this really necessary? And how many folks actually took there tortoise to the vet for an infection in their mouth? Thanks to all who REPLY!!
