When he is in his cage he sounds fine. If he did have an infection or sickness of some kind, wouldnt he have a snotty nose or puffy eyes or something to further indicate congestion or sickness?
You are absolutely right, but it is important to get them to the vet once every 6 months for a once
over.
As far as his diet, it has mainly been Romaine Lettuce, carotts, spinach, bannanas and squash. I know that this isnt the best diet and I am changing it imidiately, more grasses and cactus and greener vegitables. Please help me out on his diet as well as informing me of some further symptoms of sickness ASAP!!

Try this,
Turnip Greens
Chichory/Endive ( a good replacement crop for Romaine)
Mustard Greens
Collards
Everything else your feeding it on the list above
is not good for the desert tort, on a daily basis
or at all.
As you get it used to eating those crops, get some
small 4 X 8 trays about 1/2 inch deep, put some
sterlie topsoil in them, and get some mixed grass seed. Grow the grass in these trays and swap them
out as it eats it.
Get a prickley pear for it, they love them.
Slowly introduce more grass, less vegtable produce.
You will see a change during elimination.
