My turtles nose is falling off

I have a Flordia Softshell that is about seven years old and the other day I noticed his nose looked like it was deteriating. By this I mean that the skin is coming off and it is bleeding, also it looks like a peice of his nose has actually fallen off. I do not know what to do for my turtle or what may have caused this to happen. If you have any comments or suggestions please write me back.
