First things first - Whatever the substrate is, get rid of it. Sounds like you probably have him on bark? In my experience, Bark = Dead Iguana. Newspaper has always worked just wonderful for our iguanas...

But back to the problems. If it was choking on substrate, it most likely ingested some. This could cause a blockage (impaction), and would explain the lack of pooping.

Then you mentioned something hanging out of its vent. Is this substrate that has passed almost all the way through, or is it an everted hemipene? Either way, it should probably get checked out.

Just FYI - I have seen an Iguana pass a white tube sock, a plastic grocery bag, and a latex glove. The worst I saw from these objects was some weight loss, and painful pooping. Consider those iguanas lucky though, as they probably should have been dead. There was an old issue of Vivarium Magazine that had a great article on free roaming iguanas, and the objects they often injest. The worst they mentioned was an iguana who ate a child's Tutu (skirt). The iguana had to be opened up, and the tutu removed. Shortly after, the iguana had a worse inpaction, as the owner decided to keep it in a cage after that, only on corn cob bedding. That impaction killed the iguana.

: Last night my girl friends iguana was choking on his substrate, by the time we found him he was half gone. I got everything that I could out of his mouth, everything that I could see even. His air way appeared to be free of any obstructions and he was taking full deep breaths. But ever since then he's been very weak and sluggish. It also appears that he might have some sort of a hernia... Excuse my ignorance in reptile anatomy but he had a little something poking out of his 'waste removal' area. It is just barly enough to see anything, but it seapped fluid most of the night. He also was unable to pass anything in a bath today. (which almost always happens) I guess to sum it up, is he injured? can we push this piece back in with care? or what? Just a couple of nervous and scared iggy parents.
