My last iguana rescue had a similar condition-- spread over his entire tail (if it is the same, it will spread!) I tried applying antibiotics but in the end, it took 3 weeks of antibiotic injections in the back to stop the spread. I guess the infection was taken care of at that point but he always had horrible scars from it.

Get rid of your heat rock first. Make sure your ig is getting a calcium rich diet and UV exposure. Make sure his living conditions are pristine--his environment should be sterile with no fecal matter getting on his wounds. Seek a vet to get the antibiotics as suggested below (plead your case and try to work something out). Good luck.
