Still confused on heating and lighting

I've read Melissa's care sheet+all the links at her site.
However I'm stil locnfused. For lighting+haeting, should I use some sort of ceramic heater with a uvb light? Or some kind of full spectrum light /w a uvb light? Im soo confused. Also, over a metal screen top... how would i go about with: mounting/placing/straigt down laying on top; the lighting+ heating withought having fire problems? What do you guys use and how is it set up? thanks
