Well, from numerous emergency igs I saw while working with the exotic vet....
My first concern would be getting those backed up urates out of there. Time for a massage! We've done this many times with MBD igs who had lost muscle control of their hind end. You basically want to use your thumbs on his lower abdomin and firmly push down, like you were kneading dough. You'll be able to feel any blockage (hopefully). Just keep pushing - you'd have to be Hulk Hogan to rupture anything. I'm sure others on this forum have had to do this at one point.
From a holistic point of view - he sounds like one majorly stressed iguana. I know you want to do everything possible to keep him alive, but how about backing off a bit. By all means tip him forward and let him expel that "loose" liquid. Let him rest in comfort and seclusion. Maybe let him go for 24 hours without bothering him. I'm sure others feel differently and have had different experiences with critically ill iguanas. I myself have done enough emergency care and seen enough emergency animals to form the opinion that it is better for very sick reptiles to have minimal contact with us. We administer the meds, we leave them alone in the warm dark.
6 1/2 years for an iguana with kidney damage is pretty good. Usually the ones we saw only made it to about 3.
You may also be complicating things with the mix of substances Q is getting - Lasix, Baytril, AlternaGel, Flagyl and Calciquid. Lasix is contraindicated for use in humans also using some antibiotics. Lasix also depletes potassium in the body, which is very important to maintain heart function. AlternaGel is not recommended for patients with edema and may cause hypophosphatemia. This information is extrapolated from human medicine, which in this case should be similar to reptile medicine.
Q may be in congestive heart failure - you mentioned the liquid around the lungs.
Obviously I'm not a vet, and I'm not holding Q in my hands. And I probably didn't really give you anything helpful. But I do want to give you my prayers and hold your virtual hand through this crisis.
