Hi Sean...and welcome!
cage size...ya know the bigger the better right? A 6x4x3 is fine to sccomodate an adult iguana. Even placed on its side with a cabinent type thing under for added height...Igs like being high up right? three sided wood, melanime (humidity tuff on the mel.), with vents...vinyl hardware cloth front doors or plexiglass with holes drilled in it for added ventilation.Maybe have removable plexi-doors for retaining winter & night heat...and a hardware cloth door set up that hinges on so you can roll it outside during day for free uvb! I keep my males outdoor enclosure heated with a ceramic heater($13.00 at most poultry feed/supply store) and infared red heat lamp at night. Also I cover the front double doors with a quilted moving blanket that fits snug over the enclosure to help retain heat when night time temps get too low.

Lettuce: I would continue to feed him lettuce but make sure it is chopped small & mixed with other types of dark leafy greens like collards, mustard , dandelion then gradually extinguish the lettuce..till its eating nutrient dense foods. At this point its important that he eat...even if its bad for him...that's why need to gradually extinguish the junk and replace with the good stuff...don't wait it out with him...too dangerous.MBD is slow & progressing...feeding small amount of lettuce to keep him eating is not doing any immediate damage! but ya need to eventually get rid of it completely.I wouldn't make any drastic changes in diet YET! Add oranges or papaya & diced squash acorn, butternut...nuke it for 5 minutes or bake for 20. till semi soft.Add a bit to salad..or throw in some diced tofu (lots of calcium in it)Igs like foods that are colorful and aromatic. The contrast in colors will attract him to eat.Fresh water always available.Put some pieces of mustard or collard greens in it to attract him to drink.

Then ya gotta keep Melissa Kaplan's website handy. She had cage size dimensions and diet suggestions and a ton of info. She is the best on the web as far as information & iguana care goes.
