I hate it when stuff like this happens to my snakes. From what it sounds like, your snake has failed to shed completely around the rostral scale due to a potential underlying injury. The first thing to do is to get the snake into a tub. Start soaking it regularly in a dilute soln' of chlorhexidine gluconate and water. Nolvasan is one of the better brand names out on the market. Betadine also works well. After that the scale should dislodge and you can see what you are dealing with. Chances are it is just irritation from rubbing the nose around the edges of the cage but you never know. I used to have a snake that did that too. Sometimes they rub the rostral scale off entirely. That is more common in indigos though and even then kinda rare. If it just scratch you can usually keep it clean with triple Antibiotic ointment (neosporin) and frequent short soaks in a dilute chlorhex bath. If it fails to resolve or gets worse schedule a vet trip.
