do you know which antibiotic the vet prescribed?
Also what course of shots was the sulcata on?
How many shots, and how often?

I would change his substrate, repti bark is not ideal for a sulcata.

What type of enclosure is he in?

Do you have a basking light as well as the UV light?
He needs a basking light so he can heat himself up.

The temperatures sound about right, but what about night time temperatures?
They need a cooling down period overnight, for a small sulcata around 75 degrees is a decent, overnight temperature.

Tell us more about your set up, so we can help with anything that needs changing.

Instead of a water dish he can soak in I would provide a smaller dish that he can just drink from.

I use the plastic top off a Sunny Delight bottle for my baby sulcata.
Maybe the plastic top of a coffee container or something similar would work, for a slightly larger tortoise.

Something he can drink from easily but not soak in.

I would have drinking water available at all times.

Answer the above questions and post again if you have more questions.
