It sounds very sick, possibly hexamita parva, whatever you do, don't get another turtle/tortoise and allow it to have conract with this one.

Does it have webbed feet?

If it doesn't have webbed feet it doesn't live in water.

What color is the shell and skin?

Any patterns /stripes on the shell or skin?
