This is what would cause me to think he still has a respiratory infection. I've got a 1 yr old c.b. indian star that arrived late last summer. Shortly after she arrived I noticed she kept holding her head out of her shell and up in the air. So she went into the vet and sure enough she had a respiratory infection. The first round of antibiotics didn't clear it up - it appeared to be gone but then I noticed her showing some symptoms again. The second round of antibiotics cleared up the infection.
It does sometimes take longer treatment to clear up respiratory infections in tortoises. Sometimes you need to use different antibiotics. I believe the vet can do a swab of the nasal area and do a culture and sensitivity. With my indian star, there is a reaction to baytril so we used a different antibiotic.
