shipping a 4.5' monitor. need help

I am relocating from Philadelphia to Los Angeles and was hoping you could clue me in on the best way to do ship my 4.5' monitor lizard. Should I use a cardboard box or a wooden crate? 1 or 2 inches of styrofoam insulation? Pillow cases are kind of small, any suggestions? Finally, who should I ship with. USPS, UPS, or FedEx? Thanks in advance.
