hi there, first of all you should get to a vet who deals with reptiles. normally there are ways to cure an impaction (bathing in warm water etc, medicine, but you should ask a vet how to exactly do that)

as for the substrate, here in europe, we all keep the reptiles on loose substrate. because substrate eating happens in 99% of the cases through a wrong diet (not enough vitamins, minerals), or parasites (in stomach etc..).
of course there are really some substrates you shouldnt use sharp edged sand, etc...
