First of all, congratulations. Ackies are awesome monitors. Now to the questions, and remember this is one person's opinion and not the only way to do things.
#1 We use 1/2 ground coconut (or peat moss) and 1/2 sand mix. It should be moist. We do not put deep substrate in the cages (about 2-3 inches), but we do offer a deep dig box. Remember they do enjoy digging, so give them an area to dig in, whether it be the entire cage or a dig box.
#2 A 40 gallon breeder is about the size cage we use for a single adult. A pair can be kept in a 2 foot by 3 foot cage.
#3 Our ackies do not climb, unless it is onto the basking spot. They do not have to have braches or limbs to climb on. You could try putting something in and just remove it if it isn't used.
#4 All of our ackies dig burrows to lay eggs.
#5 We do not heat the substrate. If the substrate is deep, then it will gradually get cooler the farther down you go. The monitors will choose at what depth and temperature they want to lay the eggs. I wouldn't worry about heating the deep substrate because you are only taking the choices away from the animals.
