I hate to say it, but it is highly variable. MBD is a misnomer. It is not really a "disease" but rahter a condition, which can be present for long periods of time without being detected, or a very short period of time. It all depends on exactly how difficient the iguana is in calcium and or D3.
Also, it depends on whether it is a small actively growing iguana, or an adult.
That said, babies could start to show symptoms in as short as a month if not provided with adequate levels of calcium, and a means of producing D3 (UVB) Gravid females would also be relatively quick, but as I have never seen one with MBD I could not say. Non-gravid adults would be appear "normal" for a considerably longer period of time - they do not require as extensive of amounts of calcium, and they have more reserves in their bodies to pull from before it would be noticable.
