I would take her to the vet for a checkup! Depending on how the spaying was done or whether or not the vet got everything when he went in, there could be issues. Another thing will be to look at xrays and blood work for possible answers. I know than mammals can go through what is refered to as "hysterical pregnancy" but I'm not sure if such a thing is possible for iguanas.
I would also check for possible calcium deficiency.

Another thing to look at is diet -what is she or is she not being fed. It could be the photo but she looks kinda small and underdeveloped for a three year old.
