How long have you had him?...could have been old problems. If you have a new light at the proper distance with a good diet (easy on the kale), then I think your care sounds pretty good.
It could just be a random freak injury or maybe he has a problem processing calcium. I'm not sure if I've heard of an iguana with that problem but I think I've heard of a bearded dragon with that problem before. It's probably not very likely though. All I can say is good luck at the vet. A blood test would tell you the calcium levels.
