Give the Ig a good long soak in the tub and then do like Iguana Joe suggested and scrub with the toothbrush. If that doesn't do it a vet trip is in order. The old skin cuts off the circulation and the end of the tail will die and may possibly spread infection back into the remainder of the tail.
