My first thought is to take him (or her) to a qualified vet for a nose to vent checkup. If it was too much to handle for a 10 yr-old, you have to consider that he may not have been eating right all this time and the skin "problem" is a symptom. It could very well be a sign of a vitamin deficiency. I strongly recommend you get him looked at.
