I dont know if this is relevant or not, but a friend of my brother has a mild case of scitzophrenia and it causes him to chew his nails COMPLETLY off, when he gets down to the skin he begins to chew the skin itself and he has chewed most of the skin off of his thumb right down to his wrist. Its really gross, but he feels that he HAS to do it or something beyond our comprehension will happen inside his head. Nothing theyve been able to do has stopped it, and theres no 'term' that describes or classifies his condition.
Maybe your guy has some sort of neurological disorder?
