Dog crates DANGEROUSE PLEASE be careful

For over a year now I have put Double D into the dog crate while cleaning out her enclosure. Today I put her in there and apparently twenty minutes of being in there she managed to rip off one toe nail and rub her nose raw. She has NEVER done any of this in the entire time she has been in her crate before. PLEASE PLEASE beware, I never had a problem and today we had blood all over. She's doing fine and ill keep a major close eye on her but I was thinking we were on our way to the emergency vets in the next town 40 mins away.
