Mark's right on with 2-5.... I'll jump in on 1&2 though....
I HAVE seen UV output numbers on the Active bulbs - which is exactly why we recommend them so highly. Both the Active and PowerSun bulbs are supposed to be similiar in output. I say supposed because I have NOT seen output results on the PowerSuns but DO have it on authority that they are comparable. Determining factors of UVB output are wattage, flood v. spot and DISTANCE FROM THE BULB. Decay rate on UVB is high which is the reason for keeping animals within 12" of fluorescent bulbs. Decay rate is the same with mv bulbs but with greater output there is proportional UV. At 12", a 100w Active flood will put out 50 microwattts/cm2. At 24" that drops to 12.5 roughly.
As far as bulb size, I would go with a 100w flood for a 4x3x3 enclosure. My absolute favorite combination for equipment is an mv bulb for UV/light/basking heat, a radiant heat panel to control ambient temps, and an ultrasonic humidifier for humidity. Those three purchases, with all of the associated controls, power strips, etc are going to run roughly $200 if you purchase everything at once. My recommendation would be to get the bulb first if you have to break up the purchases.
