Walter may have something more going on than blocked bowels, he might have a vareity of things, but since the bowels look unblocked, think about the muscles.... he could have strained muscles and a little effort hurts, or herniated disks and that effects the muscles....
Soak him in real warm water, when he tries to go (the grunts you hear) place your hand around his middle section and help support his muscles by applying light pressure. DON't Squeeze to much, but firm pressure.
I know this from how humans are when having strains or herniated disks.... going to the bathroom when inflammed takes almost an act of god, even stool softners do not help alot....
Also, ask your Doc about some anti-flamatories for a few days and see if that does not help
