You probably won't find any answers here, unfortunately, because it seems you have done everything right. Another vet opinion is my only offering. From your description, it seemed like renal problems, which are very common in captive iguanas, but unless your vet missed something, this is not the case, as kidneys were normal in the exploratory. Failing kidneys are usually very enlarged. I might also suggest that you request the vet to do extensive blood tests at a different lab to look for abnormalities.
