You mentioned that the abdomen appeared mainly as a milky cloud. If you the capability to do it, you may want to try "colorizing" the x-rays. This may help in defining any edges that may be hiding. We tried this trick with Mel's x-rays when she was gravid last season. Worked quite well for us actually.
If you have a scanner and something like PhotoDeluxe you can scan the x rays and then import them into Photo and add color to them. We found blue to be very helpful. It could be worth a shot as it may simply be an investment in time for you as opposed to money for you and further stress for Shirley.
If you don't have this capability a copy center may be able to help.
As far as getting another vet involved...At this point, I would say that the more educated people that see this the greater possibility of getting this solved. If the x ray colorization doesn't work and you can get the ultrasound done I'd say do that, keep a complete file of all of the results as they come in and gather as many opinions as possible.
You may also want to take a look at any possibilities of bacterial infections. Not necessarily the direction that the bloodwork may indicate but one more thing that would be very good to rule out. We just had friends go through that with their ig. They took it as far as the vet conference that took place in FL back in March/April or so.
