First & foremost you need to take this iggy to a reptile vet....fast!
Second, lose the hot rock... this is BAD for iguanas. What are the temps in your enclosure? How big is the enclosure? What is the humidity level? What is the substrate?
Third: your diet isnt sufficient. Kale & Spinach should only be fed in moderation as a little treat once & awhile. Lettuce is equivelent to water... only include it if the ig is dehydrated. A good diet consists of the following... Collard Greens, Mustard Greens, Snow Peas, dandelion greens, escarole, aruguala, watercress, carotts, butternut squash, parsnip - and a little bit of fruit likke figs, bananas, strawberries & such.
Pick up Melissa Kaplans book "Iguanas for Dummies" & read it... and put it into practice.
Also, visit www.anapsid.org to learn proper ig care. Coming here helps too.
BUT remember... YOU MUST SEE A VET ASAP if you want to do what is right & save the ig.
