Boy, with one question you have spanned a multitute of answers. Could be lighting, heating, food, sunlight, worms, nerves, or genetics. You are going to have to do some studying on your own. I put some links at the bottom that have really good information on the general husbandtry (sp) of iguanas. Hope the links work.
Calcium supplement is just not all that is needed. Without proper lighting or natural sunlight (not through a glass window) the iguana can't make good use of the calcium your giving him. Proper lighting would be a special UVB flourscent tube (not bulb) over his basking spot. Also the correct tempertures to aid in the digestion process. As a calium supplement, I just use tropical flavored tums. I crush that and vitamins, put it in a salt shaker and sprinkle on the food. Again you need to read. Don't use just any vitamins. Whole other subject.
To clear your mind about worms. Take a poop sample down to your vets for a fecal exam. Put fresh poop in a container (35mm film case) label with ig's name, and rush to your local vet. For a fecal exam most any vet can check it out. If worms are found you will need a reptile vet to prescribe the meds and doseage. If none in the area, your vet can call a rep vet for consult. Don't let a regular vet prescribe the same worm meds for cows to your ig - deadly.
Mr Brown Toes should be see by a reptile vet for an acturate diagnosis.
