If so, you could send that along to Skinny and folks could fill in blanks and print off when needed :o)

If not, I could probably make one up using a copy of a kid's medical release form :o)

One drawback tho....ahem, most of us won't be able to have an ig sitter as competent as Marie...so we probably wouldn't want to release the final decision on medical procedures to our sitter. I, of course, would trust Marie to make the right decision. And I'm NOT saying I wouldn't trust my friend Gwen but I don't think she'd want to be in a situation where she HAS to make a decision beyond going to the vet. It does make really good sense tho to have a form that authorizes your sitter to seek medical advice and then allow the sitter or vet to contact the owner for the final decision.
