Gotta remember the old "First do no harm" rule...that being said and remembering that I'm not a vet..I'm wondering if this scale problem could be a fungus problem. Has your vet tried any anti-fungal powder or cream directly on these spots?
The reason I'm asking this is because the antibiotics don't seem to be doing the trick. Now this could be, as BabyHerp suggested, because it is not the right antibiotic for the bacteria if indeed it is bacterial in nature. Also, as she suggested, a biopsy/culture would tell whether or not this is bacterial and which one and then that would direct the vet as to which antibiotic to prescribe.
Does the vet or do you think this problem is "urgent" to fix? If not, then if it were me I would try the anti-fungal route and see what happens. Before you apply anything to these spots, I would URGE you to doublecheck with the vet to be sure that it would not hurt anything.
I realize that what you are describing isn't exactly "black spots" but I'm leaving a website of Melissa Kaplan's that goes into ways to determine what "black spots" are and how to treat because it describes the fungus business.
I also think it would be WONDERFUL if you and Des can get together and let her take a look at these areas.
One other thought.....what UVB light are you using? I seem to recall some talk about one of the lights that was producing 7.0 and people who were using this light seemed to think it was causing spots to occur on their iguanas...almost like a sunburn. To my knowledge, nothing was proven to be "wrong" with these lights but folks seemed to be wary of them after so many reports. Des may remember this as I believe I read it on the Iguana Mailing List.
As long as I have rambled this much, I'm also wondering if there is any way these could be warts. They do get them :o) Something I didn't know until I was trying to help Cirsisue. I can't recall now if there would be fluid tho if disturbed. The link on this is:

Hopefully, I haven't confused you even more LOL You've spent a lot of money on this mystery and I'd like to see you get to the bottom of it without having to take out a loan! Keep us posted on what you try....we will all benefit. And I'm terribly jealous that you can get hands-on help from someone from this forum!!
