Try taking it back to the store first. Most stores will take it back (do you have the reciept) for a new one and then they deal with the company for you. Other then that contact the manufactorer for the requirements for return.

That said- was the fixture a proper fixture?? Did it handle the proper temperatures/wattages, was it made for this type of bulb.
