First, he needs a vet ASAP. An experienced reptile vet, not someone who just prescribes a broad spectrum antibiotic without knowing the problem. Unfortunately he has probably been sick for a quite a while. Chams are good at hiding signs of illness until they are almost gone. We need a lot more detailed information! How old is he, how long have you had him, what SPECIFIC type of lighting do you have, what do you gutload (list the ingredients) his insects with, what vitamin dusts have you used (you didn't mention any), how often do you dust, what temperatures (during day and during night), how often do you spray, what is the air humidity level in the cage? It may be too late if he is this sick. Problem is it could be many things at once and we can't help you rule them out without more complete info. Hope this helps.
My first guesses (based on seeing a lot of messages like this) is too warm from day to night, dehydration, vitamin deficiencies from improper gutloading and dusting, and stress from handling or too much disturbance around the cage. If he's wildcaught possibly a major parasite load too.
