I've had this happen to me with various Elaphe clutches. For me it seemed to happen to the clutches where the hatchlings were the largest. It also appeared to me that the clutches that had the largest babies in the eggs were incubating in the moistest setup. I'm not certain about it, but it sort of stuck in my mind that too much water was absorbed into the egg, helping the babies to attain a bit larger size in the egg, which led to them not having enough room to maneuver inside to properly utilize their eggtooth. Since those episodes I've always strived to have a "too little moisture" as opposed to "too much moisture" when incubating. I'm sure that arguments could be made against what I think I found but this was how I saw it at the time.
