************* FORM: ***************************************

Cut and Paste this part, fill in the information and email to
ASAP.

Delete the workshops not attending, BUT PLEASE GIVE ME A FIRST,
SECOND
AND A THIRD CHOICE IF YOU HAVE THEM . IF you only sign for one,
I will
presume that if that one does not make, you are cancelling completely,
so please give me the info.

I would still PREFER the registration form be faxed TO (956)
544-7808
or mailed in ASAP, IF POSSIBLE and any donation (if you please) to BE
MAILED IN ADVANCE, BUT the cost of materials or admissions can be
taken
care of at the site directly with the presenters. HOWEVER, totally "on
line" will work with follow up on site Friday, the 15th of Sep. at UTB.

Thanks!
