Chip.
The diet needs some improving , a larger variety of fruit and veggies, and leafy greens. Plus some live food and animal protein. You haven't told us how the turtle is being housed, indoors or out?
If it has constant access to water for drinking/soaking etc, UV lighting if indoors?
