Mike can you give us more info.

How long have you had the tortoise?
How is it housed, explain in detail type of enclosure, temperatures, substrate etc?
Has it ever been checked by a qualified reptile vet?

If it hasn't yet seen a vet and had a stool test done, I highly recommend you take it to a vet ASAP.

My guess from your short post is that it may be too hot.Or if it's a new tortoise it could be suffering from a large internal parasite load, this must be treated by a qualified veterinarian.
It could be dehydrated also, especially if it's a new tortoise.

You mentioned a heat lamp, what temperature is it both under the lamp and in the cooler side of the enclosure?

There must be a gradient, a warmer side and a much cooler side, so the tortoise can thermoregulate.
Heat lamp off at night.

But please give us more info, tell us everything you can about the set up. How long you've had the tortoise etc.The more we know the more we can help.
