The substrate might be the cause.
Constantly walking on concrete can wear down the skin on the paws, and outdoor carpeting might contribute too.

I would make sure she's not on any abrasive surfaces, that could cause the above to happen.

Did she have trouble getting up the ramp before, or is something that happened recently?
