First the weight. If it weighs .5 lbs at 4.5 inches that is not bad. If it was active when you bought it give a little time to acclimate. Stick with the leafy greens. Try romain and other lettuces. As far as the parasites go continue working with a vet. These tortoises have a reputation for being heavily parasitised.
