I would get him out of the tank and into something with more air circulation. A large rubbermaid or something.
As far as the diet, I'd cut out the corn , alphalpa sprouts and apple and add endive, turnip greens and romaine. Corn and sprouts are too high in protein and the fruit isn't needed at all.
As well as rep cal (which is a calcium supplement) twice a week i'd give herptivite which is a vitamin supplement made to go with rep cal.
Don't overdo the supplements, twice a week is fine.
Try to get him outdoors if it's warm enough.
Even Hoku (who's indoors mostly because of his injuries), isn't nearly as active as he is when outdoors.
Keep up the soakings, if he is dehydrated the vet can give hime fluids.

Check those temperatures to make sure you really are in the correct range. Not too hot or too cold either of which could make him lethargic.

What substrate are you using now Kenneth?

Finaly where do you live and is your vet experienced with reptiles?
If not, maybe we can find you another vet.
