I agree with Marissa, vet check and husbandry check.
If you haven't taken the tortoise to a vet since you got it then now would be the ideal time to do so.
The vet can do a general check-up, examine the eyes and do a stool float all at the same time.
I got a Russian on Saturday, just got back from the vet.
My Russian is infested with pin worms, he's been treated to be repeated in 2 weeks, and will go back to the vet in 3 weeks.
Can you describe your set-up as Marissa asked?
Russians can't stand damp conditions, so make sure the enclosure is kept dry.
If you were having better luck with the previous substrate I would start using it again.
But as Marissa suggested, a vet visit would be the beast idea.
