I think there have been. I know of at least one, many years ago, but it was only indirectly caused by the adder. The person died due to the anti-venom used - allergy or something. I've not personally heard of any, although I live in an area with a lot, and many dogs, including mine, get bitten. But it's very rare for people to get bitten.
